The question is impossible to answer without knowing more about the design of the bridge. There is almost no such thing as a pre-made bridge (except maybe in the military.) Every bridge is unique. Revit can model almost anything you want from it. I have been using Revit since 2003 and have never found anything it can't do if you put your mind to it.
As for reliance on pre-made families, nothing could be further from the truth. In fact, in the early days of Revit (before Autodesk) there were almost no pre-made families for anything. If you wanted a toilet, you had to make the family on your own.
Most questions like this one come from people who don't know Revit very well and are speculating on what Revit can and cannot do. BTW: I think you probably want Revit Structure to build your bridge model.

Modeling from basic 3D shapes is how almost everything is made in Revit, pretty much like any other design program. No mystery there. A family, whether pre-made or custom, is just an assembly (optionally parametric) of basic shapes that you save with a name. Any family from any source can be edited to suit the particular needs of a project. There is a pretty full complement of 3D tools: extrude, blend, sweep, swept blend, revolve, and the void forms of these for drilling holes and other sculpting. You can also draw, if needed, a simplified 2D representation of a complex family so it doesn't show up as a dense blob of ink in, for instance, a plan view.
I have documented a few in revit! @neil-barman yes ofcourse you can model anything in revit. Model in place is the best tool of revit to familiarize yourself with - making 1 off custom families via extrusion, profile etc. etc.

Can't you just model it up from basic 3D shapes, the way you can in Vectorworks or Sketchup?
Or does Revit only allow you build based based on existing tools and pre-made "families"?
I have seen some amazing things morphed out of the stair railing tools (not stair rails, think outside the box and be creative). Perhaps that one tool can assist in getting you there.

Haven't done it with Revit but with ARCHICAD its pretty straightforward.
Yes, Revit does a lot, but what I have noticed of late is that people don't normally go beyond the box, model bridge is basic thing one can model in revit. I think the poster of the question is not fully revitalized.
I decided to investigate the Google query on bridge design a little more thoroughly and discovered that Revit Structure actually has an extension app for bridge design. Check this link out:

Stair railings have nothing to do with it. Just draw the bridge as designed. Go to Google images and search for "Revit bridge design" and you will see the possibilities.
